What is the Relief Care Program?
The Relief Care Program is a service that provides relief to a family and /or other caregivers who care for an individual who is dependent physically or mentally and require care and supervision on a daily basis.
What Services Does This Program Provide?
Some of the Services Include:
· Registered Nurses, Licensed Practical Nurses and Resident Attendants 24/7
· In-house meals
· Clinical Dietitian
· Physiotherapy & Rehab Support
· Hair Care Services (Nominal Fee)
· Laundry and Housekeeping Services
· Recreation and Social Activities
· BBQ and special events
How Do I Access The Relief Care Program?
To be admitted to our Nursing Home under the Relief Care Program, follow the steps listed below:
1. Have your physician complete the form entitled “Physical Examination and History Form”. Bring the completed to the nursing home prior to admission date.
2. Complete the form entitled “Application or Admission Relief Care Program”, ensuring that you answer all of the questions on the form. Return this form to the nursing home.
3. After the nursing home has received the two above forms a representative from the nursing home will contact you to discuss care needs.
4. Read carefully the document entitled “Agreement to Admission to Relief Care. You will be required to sign this Agreement prior to being admitted to the program.
5. If you qualify for admission to our nursing home under the Relief Care Program, you will be notified to confirm your admission date.
6. Physician must fax to the home any prescription required prior to admission.
7. The family must pay the cost of care upon admission
On The Day of Admission You Should Bring:
Five (5) changes of clothing
Wheelchair, walker is applicable
Personal toiletries
Anything that will make you comfortable
